Triangle Away suddenly stopped working on my device

I Burned a lot of versions on my device last six months.
When burned Version 4.1.2 (Polish version) and ran again ( as always before ) the Triangle Away But the my galaxy still shows me that the
version has been changed (THE FLAG IS 1).

I re-installed the version after hard reset - but still same ... still see that the version has been changed.
I mad again the hard reset and installed the preview version .... and again i see that the version has been changed !

What to do ?
How i fixing it up ?
